You have the right to receive a “Good Faith Estimate” detailing the expected costs for your healthcare services.

Under federal law, healthcare providers must provide patients who are uninsured or not utilizing insurance with a clear, written estimate of the total expected charges for scheduled healthcare services or items. This estimate includes fees for chiropractic care, examinations, therapies, medical tests, equipment, and related expenses.

Your Rights:

* You are entitled to receive a Good Faith Estimate in writing at least one business day before your scheduled service or item.

* You may request a Good Faith Estimate from our office or any other healthcare provider before scheduling a service.

* If the billed amount is $400 or more above your provided estimate, you have the right to dispute the charges.

We recommend that you retain a copy of your Good Faith Estimate for your records.
